Grievance Procedure

SUBMITTING AN OBJECTION

When submitting an objection, the following procedure must be followed:

• Complete the Submission Form for Lodging an Objection here explaining the details of the objection.  Include all relevant facts and any supporting documents. Reference the specific DONA International Standards of Practice or Code of Ethics alleged to have been violated if known and understood.  Objections filed anonymously are unacceptable and cannot be processed.

The form must also be submitted to American Family Center, LLC at PO Box 3004 Gilbert, AZ 85299.
American Family Center, LLC must respond within 15 business days upon receipt of the complaint.

• Email the completed Submission Form for Lodging an Objection and supporting documents to

GrievanceChair@DONA.org with a copy to DONA@DONA.org. 

•The objection must be received no later than

eighteen (18) months after the date of the alleged occurrence.

If the student complaint cannot be resolved after exhausting the Institution’s grievance procedure, the student may file a complaint with the Arizona State Board for Private Post-Secondary Education. The  student must Contact the State Board for further details.  The State Board address is:
